Ankle sprains are predictable on this island

Key West is not a particularly forgiving surface to walk on. The cobblestones on Duval Street, the uneven pavement in the older neighborhoods, the boat docks and marina gangways, the beach volleyball courts and pickleball complexes. All of them produce a reliable supply of rolled ankles each week. Most of those sprains are what they appear to be: ligament strains that heal with rest, ice, compression, and elevation over several days. Some of them are not sprains at all. They are fractures that happened to look and feel like sprains, and the only way to know the difference is an X-ray.

Why some ankle injuries need imaging

The ankle is a complex joint with multiple bones, and fractures can occur in places that do not always cause dramatic pain. An ankle that is sore and swollen might have a fracture of the lateral malleolus, the medial malleolus, or the base of the fifth metatarsal (the small bony bump on the outside of the foot that commonly fractures when the ankle rolls). Some of these fractures allow limited weight-bearing, which can create a false sense that nothing serious happened. Treating a fracture as a sprain (wrapping it, walking on it, going about your vacation) can displace the bone and turn a manageable injury into a significantly more complicated one.

What the clinical guidelines say

The Ottawa Ankle Rules were developed specifically to help clinicians decide which ankle injuries need imaging. They recommend X-ray evaluation when there is pain at the tip of the lateral malleolus (the bone on the outside of the ankle), pain at the tip of the medial malleolus (the bone on the inside), pain at the base of the fifth metatarsal, or inability to bear weight for at least four steps at the time of injury or in the clinic. These rules are well-validated and widely used. If any of these conditions apply to your ankle injury, imaging is the appropriate next step, not watchful waiting.

Go to the emergency room for an ankle injury with visible bone deformity, a limb that is cold or pulseless below the injury, an open fracture where bone is visible through the skin, or significant numbness or weakness in the foot or lower leg.
